

Positive Transitions is a behavioral health counseling and mentoring program
sponsored by ETI for Medicaid eligible youth and their families. The Positive
Transitions Program is NOT a part of the ETI Gang Project, but does provide
many court-required services.
Philosophy
Positive Transitions approaches each person as a unique individual with
strengths and abilities, and needs and limitations yet, with potential for growth
and development.
Positive Transitions assumes a holistic approach to the individual, recognizing
the interrelationship among the physical, social, emotional and environmental
aspects of well-being.
Positive Transitions promotes positive attitudes toward self-image by
encouraging and modeling positive/successful relationships with peers and
adults.
Positive Transitions works to empower all persons served in making a positive
transition into a successful, productive citizen of the community.
Individual Counseling is a method of treating behavioral health disorders using
the face-to-face one on one method of interaction between a clinician and a client to
promote emotional or psychological change to alleviate the mental disorder. Individual
therapy must be provided in an appropriate, private, confidential setting. The therapy
must be goal directed utilizing techniques appropriate to the treatment plan. Individual
counseling is provided by a Mental Health Professional (MHP) as designated in
standards set by the Oklahoma Health Care Authority.
Group Counseling is a method of treatment of mental disorders using the interaction
between a therapist and two or more clients to promote emotional or functional change
to alleviate the mental disorder. The focus of the group must be directly related to
goals and objectives of the treatment plan for each client. Group counseling does not
include social skills development activities and must take place in an appropriate,
confidential setting. Group counseling is provided by a Mental Health Professional
(MHP) as designated in standards set by the Oklahoma Health Care Authority.
Family Counseling is face-to-face interaction between a clinician and family to
facilitate emotional, psychological, and behavioral changes and promote successful
communication and understanding. Family counseling is provided for the benefit of the
identified client, and is specified as a component on the clients treatment plan. Family
counseling is provided by a Mental Health Professional (MHP) as designated in
standards set by the Oklahoma Health Care Authority.
Individual Rehabilitative Treatment is face-to-face service that is performed to
assist clients who are experiencing significant functional impairment due to mental
illness or behavioral issues. Individual rehabilitative services are provided by a
Behavioral Rehabilitation Specialist (BHRS), and focus on increasing the skills
necessary to perform activities of daily living and functioning in the community.
Services will be for the reduction of psychiatric and behavioral impairment and the
restoration of functioning consistent with the requirements of independent living and
enhanced self-sufficiency. This service includes educational and supportive services
regarding independent living, self-care, social skills (Re. development), life style
changes and recovery principles and practices. Each individual rehabilitative treatment
service must have an objective and purpose relevant to the treatment plan. This
service may be
provided one on one between the client and the BHRS, or may be provided with
parent/guardian present or occasionally with only the parent/guardian for the purpose
of treating the identified clients disorder.
Other family may be present if pertinent to the treatment goals and objectives.
Mentoring- A client may be assigned a mentor to serve as a role model and as a
source of encouragement or support. The role of the mentor is varieddepending on
the needs of the client and family. The Mentor may present life skills sessions, provide
transportation services, develop and monitor recreational activities, develop and
monitor community service activities, and document required contacts and notes
into the client's record. The Mentor will be responsible for assisting clients improve their
interpersonal skills, their employability, and assist clients who are exiting from the
juvenile justice system and/or returning to traditional educational systems as needed.
The Mentor may also tutor. Mentoring services are one-on-one or in small groups.
Crisis Intervention Services are face-to-face services of an emergency nature to
resolve acute emotional dysfunction by providing assessment, intervention, and
stabilizing functions to the individual. This includes screening, planning, referral, and
documentation. After hours calls will be handled by the Clinical Director, who will
contact the appropriate person to arrange the response to the crisis situation.

standards set by the Oklahoma Health Care Authority.
Treatment Plan Development includes the evaluation of diagnostic information in
order to develop the treatment plan delineating the place of service, types of services,
the goals and objectives established with the client, therapeutic techniques, modalities,
strategies, evaluative criteria, and time lines. Providers must actively involve the client
14 years and over in the development of the treatment plan. In the case of children
under 18 years the childs family, guardian, or custodian must also be involved in the
development of the treatment plan. The treatment plan includes specific service
strategies to deal with identified problems and maximize and develop client strengths.
Each plan illustrates plans to fully integrate the client in the home and community.
When children are treated during the school year, the treatment plan must
demonstrate collaboration with the education system. A specific provider must be
named for each treatment service provided.
Treatment Plan Review is the comprehensive review of and evaluation of the current
treatment with the client. This includes modification of the plan as required. Treatment
plan review may be in the form of a multi-disciplinary staffing or at times only the
physician and the client. All treatment plan reviews include an update to the individual
treatment plan. Client participation and documentation will be clearly documented.
